Maintenance treatment of patients w/ COPD (including chronic bronchitis & emphysema) & associated dyspnoea. Spiriva Handihaler:Prevention of exacerbations. Spiriva Respimat:Add-on maintenance treatment for improvement of asthma symptoms, quality of life, & reduction of exacerbations; in patients ≥6 yr w/ moderate to severe asthma who remain symptomatic on at least inhaled corticosteroids.
Spiriva Handihaler Inhale 1 cap once daily w/ the Handihaler at the same time of day. Spiriva Respimat Inhale 2 puffs of spray once daily at the same time of day.
Hypersensitivity reactions. Not for initial treatment of acute episodes of bronchospasm ie, rescue therapy. Not to be used more frequently than once daily. Patients w/ narrow-angle glaucoma, prostatic hyperplasia or bladder-neck obstruction. May cause inhalation-induced bronchospasm. Avoid contact w/ eyes. May affect ability to drive & use machines. Moderate to severe renal impairment (CrCl ≤50 mL/min). Avoid use during pregnancy. Not to be used during lactation. Spiriva Handihaler: Contains lactose. Spiriva Respimat: Not to be used for initial treatment relief of acute episodes/symptoms of bronchospasm; as 1st-line treatment for asthma. Increased risk for adverse events eg, wheezing & breathing difficulties in patients w/ asthma due to benzalkonium Cl content.
